Antares was apparently exploded by its handlers pushing a button when trouble in functioning was detected – wanted to avoid risk of failure/explosion near populated area.
Yup, that is standard procedure whenever something goes catastrophically wrong. But the rocket was already fatally injured before the range safety officer “pushed the button”.
